Downpipes block less often than gutters themselves, but when they do, it can go unnoticed for a while since the blockage isn't visible from ground level the way an overflowing gutter is. We check downpipes as part of a standard gutter clearing visit and clear any blockages we find, since a fully cleared gutter with a blocked downpipe still won't solve the underlying water flow problem.
